Online check-in easter event 2024
Every child (individual person under the age of 18) is required to have a parent or guardian's approval for attending the plfc easter event (and explicit consent given for certain activities therein), whose emergency contact details must be recorded for use in an unforeseen circumstance.
Previously this has been handled through a Consent Form requiring completion at the front door on arrival at the event.
To expedite this process: we are now offering an optional online checkin - an electronic copy of the form which we will print off and have available at the front desk, requiring only a signature on the day.
Online checkin is not required. However, whether electronically provided (and printed by plfc) in advance or written by hand at the door, a completed and signed copy of this form must be supplied for every child attending.
plfc takes personal privacy very seriously. All required personally identifying information collected through this online form is encrypted before being stored and will be accessed only by church staff for the purpose of printing a physical copy to be signed on the day of the easter event. Any additional optional information (which may include personally identifying information) will only be used for the purposes explained below.
